Initial Assessment Includes:

  • A home visit by a Minuteman By Your Side Care Manager begins with a conversation about your personal goals
  • An in-depth assessment of your ability to live independently in the setting of your choice including:
      • Assessment of your ability to accomplish activities of daily living such as meal preparation, dressing, bathing, housekeeping, transportation or getting to social activities
      • Your ability to manage your health issues and medical appointments
  • A Home Safety Assessment will identify accident and fall risks – recommendations and resources to improve home safety will be provided
  • A review and discussion of your ability to manage your medications
  • A review of your health planning documents
  • A review of your health benefits and prescription drug coverage options by a certified SHINE counselor at the location of your choice
  • A Personalized Care Plan with recommendations, estimated costs and resources.
