Minuteman’s Care Managers are Problem Solvers.

They understand the challenges of daily living and caregiving that can often accompany aging, health issues and living with a disability. How to maintain a home or apartment, obtaining meals, transportation to medical appointments, and personal care activities are common concerns. For family caregivers, balancing the competing needs of family, work and self can often become overwhelming.

Care Managers offer expertise, advice and access to services which enable an older or disabled person to age with dignity. We also support family caregivers. For over forty years, we have been helping seniors understand their care options, secure services or find an appropriate living situation. We will:

  • Conduct a comprehensive in-home needs assessment in consultation with caregivers
  • Develop a personalized care plan
  • Assist in vendor selection, service arrangement and quality monitoring
  • Provide support, coordination and appropriate referrals during transitions between health care settings and relocation
  • Provide access to specialized consultation and training on caring for someone with Alzheimer's Disease or other dementias
  • Provide caregiver support and respite care options

Care Management through Minuteman Senior Services is available to everyone.

Following the comprehensive assessment the Care Manager will discuss the best available care options that fit within your family's resources. The Care Manager will inform you whether your services can be covered in full or in part through our state-funded home care programs.
