Memory Disorder Consultation

The Memory Disorder Program provides unique in-home consultation and training based on the Habilitation Therapy model designed by the Alzheimer's Association, Massachusetts Chapter. Habilitation Therapy is designed to maximize the independence and function of persons experiencing Alzheimer's Disease or a related disorder and create and maintain positive experiences for them. The consultation provides caregivers with the tools necessary to care for a person with a dementing illness on a day-to-day basis through education and support. Education and early intervention can enhance an individual's ability to function optimally at home, increase caregiver confidence and reduce stress on the family system.

Program Components
  • An in-home consultation by a Memory Disorder Specialist who completes a comprehensive evaluation of the client and identifies the needs of the family/caregiver and evaluates home safety.

  • One-to-one education provided on diagnosis and disease process, communication, behavior management, personal care, activities and recreation, caregiver supports and services, and community resources.

  • An individualized plan of care mailed to the caregiver within three weeks of the initial consultation. The plan of care will include recommendations regarding home safety, managing challenging behaviors, accessing appropriate resources in the community and identifying and planning for future needs.

  • Follow-up phone calls and/or visit(s) to provide support and assistance in carrying out the plan of care.

The service is available to all residents of the sixteen communities served by Minuteman Senior Services. Please call for fee inquiry.
