Care Planning & Management

Whether you live nearby or at a distance, caring for someone can be difficult, stressful and time consuming. A Care Manager can offer expertise, advice and linkages to services to enable an older or disabled person age with dignity and give you peace of mind.

At Minuteman Senior Services we have been helping caregivers for over twenty years to clarify their options, secure services or find an appropriate living situation. Care Management through Minuteman Senior Services is available free of charge for elders who qualify for any of our subsidized home care programs. Click here for a description of Home Care programs.

For all others Care Management is available on a private pay basis through our Senior Options program that offers the following:

For an elder or disabled adult living at home we can:
  • Conduct an in-home needs assessment in consultation with caregivers;
  • Describe the best available care options that fit within the family's resources or available through state home care programs;
  • Assist in vendor selection and service arrangement;
  • Monitor care quality and report to the family;
  • Assist with selection/transition to assisted living or nursing facility;
  • Assist with completing applications and claim forms;
  • Provide specialized consultation and training on caring for someone with Alzheimer's Disease; or other dementia's;
  • Serve as a liaison with health and social services providers;
  • Escort to medical appointments and report findings to the family;
  • Make referrals for legal, financial, or mental health needs.

For an elder or disabled adult living in a facility we can:
  • Consult with facility staff regarding a suitable care plan;
  • Monitor the quality of care through regular visits;
  • Report to the family on changes in status or care issues;
  • Serve as a liaison with facility staff, a resident and/or physician;
  • Assist with Medicaid applications and other paperwork;
  • Coordinate services upon discharge;
  • Consult with facility staff regarding special issues of elders with Alzheimer's Disease and other dementia's.

Our Private Care Managers are seasoned professionals who carry credentials in nursing or social work. Minuteman Senior Services is a non-profit organization, which has no ownership interest in any direct service or facility.
